Senior Social Workers - Care Coordination Team
HOURS: Temporary 12 month contract with possibility of extension fulltime and parttime available Monday – Friday shifts available between 8am – 8pm.
TERMS AND CONDITIONS: Level 7 as per the Ramsay Health Care WA Hospitals Health Services Union Enterprise Agreement 2008
The Care Coordination Team will provide comprehensive Allied Health assessment to improve discharge planning and quality of care in the Emergency Department.
PRINCIPAL RESPONSIBILITIES TO INCLUDE (but not limited to):
* Work as part of a Care Coordination team to provide an effective service to JHC Emergency Department
* Plan and undertake Social Work interventions using established Social Work methods.
* Provides advice and support to medical, nursing and allied health professionals within JHC regarding social aspects of planning for ongoing care for referred clients.
