Social Worker - Maternity Leave Relief

The Social Worker - Florence Syer Unit is responsible for assisting patients and their families to make decisions regarding ongoing care options that best reflects the patient's needs and are consistent with best quality care.

The Florence Syer Unit is a 30 bed step down unit which provides aged care for patients awaiting nursing home or hostel placement and a safe environment to recover from acute or chronic illness or surgery prior to returning home.

The role is responsible for assisting patients with the residential placement process, counselling and discharge planning for inpatients within the Florence Syer Unit. This position works within the Case Management team, working 4 days a week for 7.6 hours per day for up to 12 months. This position would be available from July 2010

The successful candidate will meet the following criteria:

Essential:

* Degree in Social Work

* Demonstrated ability to efficiently manage case load demands

* Highly developed interpersonal skills and ability to communicate with a wide range of customers

* Demonstrated ability to provide effective customer focused services

* Commitment to professional development and maintenance of current evidence based practice

* Flexible positive approach to work

* Demonstrated practical skills in managing discharge, seeking feedback from relevant personnel and facilitation of group case conferences.

* Demonstrated ability to manage conflict

Desirable:

* Membership of AASW

* Experience working in a private hospital setting

* Recognised postgraduate professional qualifications
