Social Worker - Jindabyne Community Health Centre

Position Summary
This position will work as 0.5FTE Social Worker based at Cooma Community Health Centre and 0.5 FTE Integration Coordinator position based at Jindabyne Community Health Centre. It is responsible for providing a social work service to inpatients and community clients in Cooma and the surrounding area and working within a HealthOne NSW model for clients in the Jindabyne and Snowy Mountains area.
Part of this role involves working to maintain and develop partnerships within other agencies and groups to support clients in their communities.
Selection Criteria
Qualification in Social Work and eligibile for membership of the Australian Association of Social Workers.
Demonstrated experience and/or ability to work with the aged, individuals and families covering a broad range of social issues.
Demonstrated knowledge and experience in case management, coordination and advocacy.
Demonstrated skills in psychosocial assessment and intervention.
Demonstrated high level interpersonal, written and oral communication skills and an ability to negotiate and resolve conflicts.
Demonstrated organisational skills including the ability to prioritise, meet deadlines and work with minimal supervision.
Demonstrated knowledge of Primary and Community health phiosophies and frameworks and the ability to work within them.
Computer literacy and the ability to use computer base statistical reporting and client management systems.
Evidence of a class C driver's licence, the ability and willingness to travel for work purposes.
Principal Responsibilities
Work as part of an interdisciplinary team of health professionals to coordinate and provide services and programs to identified health priority groups utilising client centred goals to enable better health outcomes for them.
Maintain a balance of prevention, early intervention and interdisciplinary team management of client centred goals.
Ensure that priorities for the service reflect NSW Health State Plan priorities and relevant GSAHS plans.
Participate in intake, case work, group work, community development and health promotion.
Provide social work intervention for Cooma Hospital inpatients as required and support discharge planning for complex clients.
Liaise with appropriate agencies to ensure effective, appropriate and collaborative client care.
Regularly review the progress of clients through case management, care plans, satisfaction surveys and case reviews.
Provide and support the education and training of staff, students and relevant community groups.
Work as part of the psychosocial team to determine priorities for service delivery.
Maintain a high standard of written communication, with client records and documentation, in accordance with NSW Health policy and guidelines.
Actively participate in clinical supervision and performance appraisal.
Take responsibility for maintaining professional knowledge and skills relevant to current social work practice.
Participate in community health, counselling team and professional meetings.
Provide advice to Cooma Health Service Manager on any issues or problems associated with the performance of the role.
Comply with Child Protection and Domestic Violence policies & procedures and attend mandatory training.
Adhere to the ethical standards of social work as laid down by the Australian Association of Social Work.
