Generalist Counsellor/HealthOne Integration Coordinator

Position Summary
This position will work as 0.5FTE Social Worker based at Cooma Community Health Centre and 0.5 FTE Integration Coordinator position based at Jindabyne Community Health Centre, providing counselling services to target populations within these communities. This includes assessment, case management, referral, support and advocacy for clients.
Selection Criteria
Tertiary qualifications in Social Work or Psychology or recognised counselling qualifications providing eligibility for membership of an accredited and professional associaton (eg. AASW, APS).
Demonstrated successful experience in counselling, case management and group work.
Demonstrated ability to work effectively in a multi disciplinary team within an evidence based framework.
High level interpersonal, written and oral communication skills and an ability to negotiate and resolve conflicts.
Demonstrated organisational skills including the ability to prioritise, meet deadlines and work with minimal supervision.
Demonstrated knowledge of Primary and Community Health philosophies and frameworks and the ability to work within them.
Demonstrated ability to work with computer based applications including word-processing and email.
Evidence of a current NSW Class C driver’s licence and willingness to undertake reasonable travel for work purposes.
Principal Responsibilities
Work as part of an interdisciplinary team of health professionals to coordinate and provide services to clients by undertaking comprehensive assessment and intervention support and advocacy.
Ensure that priorities for service reflect the priorities and targets of the NSW Health State Plan and relevant GSAHS plans.
Establish client centred goals within timeframes and determine suitable referral, case management, counselling management and strategies.
Liaise with and refer to other health professionals and services, both internal and external to health to ensure effective, appropriate and collaborative client care.
Provide clinical intervention for Cooma hospital inpatients to support discharge planning in complex cases.
Maintain formal links with GSAHS HealthONe team and participate in Area and local HealthOne initiatives as appropriate.
Maintain client records in accordance with NSW Health policy.
Regularly review the progress of clients through case management, care plans, satisfaction surveys and case reviews.
Actively participate in regular clinical supervision.
Supervise students as required, to assist in the development of the counselling service in accordance with the requirements of the respective professional association.
Participate in continuing education and professional development opportunities.
Participate in performance appraisals as a quality improvement strategy.
Comply with data collection requirements, electronic or paper based.
Participate in relevant local and Area meetings as appropriate, activities and programs as determined in consultation with the Allied health manager.
Participate in research and evaluation programs as appropriate.
Provide advice to the Allied health Manager or other designated line manager on any issues or problems associated with performance of the role.
Attend and contribute to in-service programs.
Be aware of and comply with the Occupational Health and Safety Act 200, associated regulations and GSAHS Policy and Procedures.
Comply with Child Protection and Domestic Violence policies and procedures and attend mandatory training.
Undertake reasonable travel in accordance with the duties of this position.
