Allied Health Assistant: Goulburn Transitional Aged Care Program

Position Summary
The Allied Health Assistant provides assistance and support to Allied Health staff in the provision of therapy services to clients of the Goulburn Transitional Aged Care (TRACS) Program, within Goulburn Health Service. The allied health assistant will work under the direction of the program coordinator, treating Occupational Therapist, Physiotherapist or other Allied Health Professional and will provide clinical interventions designed to assist clients to reach their rehabilitation goals. The allied health assistant will operate as a member of the multidisciplinary team with line management from the TRACS Team Co-ordinator.
Transitional Care is a program funded by both State and Federal Governments. It is designed to provide short term support and active management for older people at the interface of the acute/sub-acute and residential aged care sectors. It is goal oriented, time limited and targets older people at the conclusion of a hospital episode who require more time and support in a non hospital environment to complete their rehabilitation, optimise their functional capacity and finalise and access their longer term care arrangements.
This position is a 12 month temporary position. There may be the potential for an increase in hours and scope of clinical work if the Transitional Aged Care service combines with a similar program (Community Acute and Post Acute Care) under an Ambulatory Care Service program for the Goulburn Area.
Selection Criteria
Relevant qualification in Allied Health Assistance and/or commitment to work towards a recognised Allied Health Assistant qualification.
Experience working in a health or residential care setting and an interest in rehabilitation and community health.
Demonstrated ability to work co operatively within a multi disciplinary team.
Demonstrated ability to communicate effectively with staff; sound verbal and written communication skills.
Proven ability to meet deadlines, prioritise workload and to work with minimal supervision.
Demonstrated high level computer literacy skills.
Current motor vehicle drivers license 1A and willingness to travel for work purposes.
Principal Responsibilities
Carry out a limited range of clinical tasks and client interventions strictly as directed by, but without the immediate supervision of, a therapist.
Following a comprehensive multi disciplinary assessment and development of a care plan, the Allied Health Assistant will, under the direction of the treating therapist, assist with the provision of home based rehabilitation programs. Activities may include but not be limited to - physical retraining and mobility programs; personal, domestic and instrumental activities of daily living retraining; equipment provision; reinforcement of communication strategies; other goal focussed client activities.
Reinforce the principles of rehabilitation and support patient centred goal setting, goal attainment and care.
Provide timely feedback on client progress and / or client issues and actively participate in multi disciplinary team discussions. Liaise with all members of the care team, including non-health providers, as appropriate.
Assist with the co-ordination of an Allied Health Equipment pool including assistance with deliveries and retrievals, cleaning and maintenance and appropriate recording procedures.
Maintain accurate and timely records of client intervention and outcomes in client medical records.
Provide clerical and administrative support to Allied Health staff working within the TRACS Program including the coordination of clinical appointments, receiving telephone enquiries, photocopying and basic computer operations.
Support the implementation of quality activities within the program as required, including participation in the ACHS EQuIP Program for the Goulburn Health Service.
Comply with Child Protection and Domestic Violence policies and procedures and attend mandatory training.
Other duties as directed by the manager.
Undertake reasonable travel in accordance with the duties of this position.
